This form is to be used to request a microchip for your pet. The cost is $10 and it will be registered with the info you submit for 1 year for free. You will receive an email that it is done. After the 1 year, you should receive another email reminding you to renew for around $20 for life. If, for some reason we are unable to insert the microchip, the cost is only $5 and you may have your vet insert it for a small fee, usually around $5 and we will still register it for the 1 year.
